Neurology is a specialty concerned with the diagnosis and treatment of disorders of the nervous system: the brain, spinal cord, nerves and muscles. Treating the problems of all adult nervous system problems are provided in our clinic. We will work with your primary doctor to help coordinate your care, testing, and work through treatment issues with you on an individual basis. We promote healing through both traditional treatments such as appropriate medications but also through patient education, therapy, lifestyle changes and recommendations of alternative treatments as appropriate. The clinic is located in Seattle, Washington near Ballard Market and Swedish Hospital.

Patient information

Please arrive 15 minutes before your scheduled appointment time to complete paperwork so we can make copies of your insurance card and driver’s license to include in your file. Please note that our patient registration, privacy information, medical release, and medical history forms are available for download below so that you can complete them prior to your first appointment. If you have seen another doctor or have had tests performed to try and diagnose it this issue, please provide us with contact information for the medical office so we can get copies of the test results for our records. Please bring a complete list of medications, as well as the dosage, that you are currently taking.

Your first appointment

During your initial appointment with the doctor, he will go over the history of the current problem as well as your medical history. This will be followed by a full neurologic examination. The doctor will then discuss his findings with you and make recommendations for any tests, suggest medications, or therapy that would be appropriate for you. A follow-up appointment can be made based on the recommendations made.

Co-pays are due at the time of your appointment. Cash and check only.
